To provide students with quality service, UScampus and the policy underwriter, ACE American Insurance Company, have arranged the services of Beechstreet PPO. Through Beechstreet's networks, students will receive the following benefits:

1. Easily locate a provider/hospital contracted with Beechstreet within the area you reside.

2. Most providers contracted with Beechstreet will process the claim directly and not request full prepayment. However, a few providers will insist on prepayment. In this situation, please follow the claim instructions in order to receive later reimbursement from the claim administrator.

3. Beechstreet providers accept the reduced Beechstreet fee schedule which saves the students money on their 20% payments.

Section 1: What type of plan do you have? Please select “Health Benefits” from the pull-down menu. 

Section 2: How do you want to search? Select your preferred search radius and enter your address, or enter information to search by provider or hospital. 

Section 3: What type of providers do you want listed? Identify the specialty desired and click “SEARCH”. A list of providers with addresses and phone numbers will be made available. You are encouraged to select “Primary Care” for your first visit unless you require specialty treatment.

For your convenience, a link to the Description of Coverage (DOC) and the Claim Form are available. We suggest that you print copies of the DOC and Claim Form and that you bring these copies with you to your appointment with a provider in order to facilitate the acceptance of the insurance program and the processing of the claim
