Claim Procedure:
When you visit a doctor/clinic/hospital, you will need to present your insurance ID card and co-payment. The doctor’s office will process your medical expenses with your insurance provider directly. Sometimes, the hospital or clinic may prefer to send the bill to you directly. In that case, just fill out the claim form, attach all
original bills and mail them to the address on the form for processing. Please read the instructions on the claim form carefully before you fill it out. Photocopies of the claim form are accepted. Incomplete or incorrectly completed forms may result in the delay of your reimbursement. You should keep a photocopy of all documents you send out.
Change of Contact Info:
If you have moved or if any of your contact information has changed, please update your information by going to www.uscampus.com and click on the “Health Insurance” tab (yellow one on top), and then click on “Change Personal Profile” (in the “Details” menu box).
Insurance Renewal:
Please be aware of your coverage end date. Remember to renew your insurance two weeks prior to the end date on
www.uscampus.com. To renew, click on “Health Insurance”, then “Renew Health Insurance” in the menu box. Login with your name and AIEF ID (1000xxxx). This number is emailed to you and written on your insurance card. If you forget your ID, please email
info@aief-usa.org. If there is a gap between your insurance coverage periods, you may be subject to the pre-existing condition clause.
Insurance Termination:
If you need to terminate the insurance policy for any reason, please note that a written notice to AIEF requesting the termination is required 30 days or more prior to the termination date. Mail, fax, and email requests are accepted. Please include your full name, plan you are under, the date and reason for termination in the notice. A prorated refund of any remaining portion of the premium, less a $30 processing fee, will be refunded if no claims have been paid during the policy term. If any insurance claim has already been paid prior to termination of the policy, no refund is available. Please allow 4-6 weeks for processing the refund check.
